Claim form

A claim form is the document you submit to ask for your share of a class action settlement. It is how the settlement administrator confirms that you belong to the class and works out what you may receive.

Most claim forms ask for basic details such as your name, contact information, and information about your purchase or experience. Some also ask you to upload proof or to choose how you want to be paid.

Claim forms have a filing deadline, and claims submitted after it are usually rejected. You can normally file directly on the official settlement website for free.
